Anutra Medical’s Q1 2017 Promotional Terms

All promotions listed below are valid from January 1, 2017 through March 31, 2017.

How System Activation and Training Works:

1. A customer places an order for an Anutra Starter Special.

2. Once customer receives Anutra Starter Special, they are to activate warranty on the system. There is an orange sticker on top of the Anutra Starter Special with instructions for activations. With activation questions please call 1.844.268.8721.

3. Upon activation, the customer is shipped a free Anutra Training iPad. The iPad comes preloaded with training modules and product evaluation survey. The office staff, particularly anyone using the system, is to go through the training.

4. After training has been completed along with the 15 product evaluation surveys, the iPad is to be shipped back to Anutra Medical with a pre-paid shipping label. The iPad must be returned within 30 days of receipt.

5. The customer will receive a $125 rebate once the product evaluation surveys are completed and the iPad is returned. The $125 Implementation Rebate will be sent via check to the practice.

In addition to the implementation rebate, the customer is also eligible for the following promotions.

Additional Promotions on Starter Special

A customer may choose from one (1) of the following three promotions in addition to the above Implementation Rebate:

1. $125 Additional Cash Rebate
2. (1) Anutra Buffering Essentials Sleeve; value of $225
3. (2) Boxes of Anutra Syringe; value of $100

Starter Special Promotion Details: The option of one of the three above are for those who purchase the Anutra Starter Special (#127-3017). The offers above are only valid on new, first-time customers purchasing the Anutra Starter Specials between January 1, 2017 through March 31, 2017. To be eligible for the special above, a customer must purchase an Anutra Starter Special and complete a full training with the Anutra iPad Training Platform and return the iPad to Anutra Medical. A full training on the Anutra iPad is defined as completing all the training modules and their respective quizzes as well as completing 15 product usage evaluations on the iPad Training Platform. The training must be completed in full to be eligible for the rebate. If a customer has previously received a Starter Special, they will not be eligible for this offer. The cash rebate will be mailed to the office in the form of a check, no later than June 30, 2017. If the option of free product is selected, the manufacturer, Anutra Medical, will ship directly to the customer, no later than June 30, 2017. This rebate form must be completed in order to receive any of the aforementioned offers. Not valid with other offers. Void where prohibited.
